Before you start completing the Agreement & Parenting Plan Order Designating School District, make sure you have the following information ready:

Case & Court Information

  • Court Name (Family Division location)

  • Case Number (if already assigned)

  • Case Name – In the matter of [Petitioner] and [Respondent]

Party Information

For both Petitioner and Respondent, gather:

  • Full legal name (first, middle, last)

  • Current address (street, town/city, state, ZIP code)

School Designation Information

You’ll need to determine and clearly state:

  • The school district the child(ren) will attend

    • Example: Concord School District or Manchester School District

  • Whether the school district chosen is based on the Petitioner’s or Respondent’s residence

  • Legal residence statement: confirm which parent’s residence will be used for school attendance purposes

Child Information

For each child, include:

  • Full legal name

  • Date of birth

(Make sure names and dates exactly match those on other court documents.)

Legal Residence & School District

  • The legal residence where the child(ren) will be considered residents for school purposes

    • Example: “The legal residence of the child(ren) is in the school district where the Petitioner resides.”

  • Indicate which parent currently resides in that district

Transportation Arrangements

Explain how the child(ren) will get to and from school, including:

  • Who is responsible for daily transportation (Petitioner, Respondent, both, or other)

  • Any special details about pickup/drop-off schedules or shared responsibilities

  • If applicable, confirm understanding that the school district is not responsible for transportation outside its boundaries (per RSA 193:12, II(b))

Parenting Plan Integration

Be ready to specify whether:

  • You already have a Parenting Plan in place that will be updated to reflect this agreement

    • Example: “The Parenting Plan dated March 15, 2025 will be updated.”

  • Or whether the agreement will update a previously signed Parenting Plan (if no effective date is available)
