For each child covered by this support order:
Full legal name
Date of birth
Order Details
Whether the order is agreed to by the parties or ordered by the court
Whether this order modifies an existing order, and if so:
Basis for modification (e.g., change in income, employment, or custody arrangement)
Explanation or details
Child Support
Amount of child support ordered ($ per payment)
Frequency (weekly, biweekly, monthly, etc.)
Start date for payments:
Select from listed options (specific date, other, etc.)
Who payments are payable to (e.g., Obligee or Bureau of Child Support Services)
Method of payment (e.g., direct deposit, payroll deduction, or other)
Arrearages
If applicable:
Child support arrearage amount and as of date
Payment amount toward arrearage and frequency
Medical arrearage amount and as of date
Payment amount toward medical arrearage and frequency
Alimony
Whether alimony is ordered
If yes: verify existence of a Uniform Alimony Order form
Employment & Wage Assignment
Whether the Obligor is currently employed
If unemployed: last employer details and reason for unemployment
Whether there is good cause to suspend the immediate income assignment (and explain why)
Medical Support Findings
Each parent’s reasonable medical support cost (from Child Support Guidelines Worksheet Boxes 2 & 3)
Whether health insurance is currently available and at what cost
Determine who provides health insurance (Obligor, Obligee, or neither)
Type of coverage (medical, dental, vision)
Uninsured Medical Expenses
Specify how uninsured medical expenses are divided:
Obligor’s percentage
Obligee’s percentage
Any other party’s contribution
Public Assistance or Medical Assistance
If applicable:
Whether TANF or Medicaid has been provided for the child(ren)
Dates during which public assistance was received
Total amount of assistance provided
Whether paternity has been adjudicated and if updates to the child’s birth certificate are required
Variations or Additional Orders
Any variations from the standard order
Additional agreements or court directives to include
